Once a week, you can expect to receive 14 customized meals, either picked by you or your RD to match your nutritional needs. Each week, you’ll receive your meals from the same delivery driver on a specific day that works for you and the driver’s schedule.
MTM program eligibility is based on your specific health conditions and needs. If deemed appropriate, your healthcare providers will put in a referral for you to receive home-delivered meals.
Following approval from the eligibility review, you’ll take a nutrition assessment with a registered dietitian. The RD will evaluate your dietary needs and create a meal plan tailored to your specific health needs and preferences.
Once your meal plan is built, prepared meals will be delivered directly to your home each week. Your meals will arrive in protective liners that can be stored in your freezer until you need them. If you’re not able to be home at the time of your meal delivery, your meals will be kept frozen in a silver bubble wrap bag with dry ice at your designated drop-off location.

You may qualify for MTMs if you’re a Medi-Cal member with chronic conditions, diet-sensitive conditions, or other issues that impact your ability to plan, shop, and cook meals.
Medi-Cal is California’s version of Medicaid. Medically tailored meals are covered by Medicaid health insurance for qualified members.
If you qualify for MTM, you can receive a referral from your primary care provider, health plan provider, or caseworker.
