Understanding Post-Hospital Care and Nutrition

What Is Post-Hospital Care?

Post-hospital care refers to professional assistance provided to people returning home after hospitalization.

Medical professionals and support services help people continue healing after they transition home from the hospital.

According to the Annals of Geriatric Medicine and Research, “Compared with younger adults, older adults are more likely to have various comorbidities and disabilities and typically require a longer time to recover from acute illness.”

LifeSpring Home Nutrition provides nourishing meals delivered weekly to help seniors and others recovering from acute illness or injury at home.

Why Is Nutrition an Important Part of Post-Hospital Care?

During recovery, the body needs essential vitamins and minerals to repair cells, replenish energy, and rebuild strength.

Proper meals support recovery after hospital stays by providing nutrients to the mind and body. Many people in post-hospital care use medically tailored meals (MTMs) to address any lingering symptoms or side effects related to their condition.

Who Benefits From Nutritionally Focused Post-Hospital Care?

People who benefit most from nutritional support during post-hospital care include individuals over the age of 65 and anyone who has experienced the following:

  • Disordered eating
  • Malnutrition
  • Health issues related to weight management
  • Chronic health conditions such as diabetes or kidney disease
  • A surgery or other major medical procedure
  • Prolonged or acute illness
  • Accident or injury requiring a hospital stay

LifeSpring Home Nutrition offers a wide range of meal options to meet the specific needs of people transitioning home from the hospital.

How Do LifeSpring’s Medically Tailored Meals Support Daily Living and Well-Being?

Our medically tailored meals help people manage their healthcare needs at home rather than rely on inpatient services.

Every medical condition requires specific treatments and management protocols. Diet often plays a critical role in managing the symptoms and side effects of chronic or acute health conditions.

We provide people in recovery with MTMs and nourishing meals that support their healing journey.

How Can Personal Care Help Loved Ones Recover at Home?

Your loved one may benefit from personal care tailored to their specific dietary and medical needs.

Home health support often plays an essential role in post-hospital nutrition and recovery by ensuring people have the resources and assistance necessary to navigate challenges after hospitalization. A dedicated care team tailors its services to the client’s needs rather than shoehorning them into preexisting treatment programs.

The added flexibility of meal delivery programs allows medical professionals and caregivers to adjust their services to meet individual needs easily.

Types of Nutrition-Focused Post-Hospital Care Services

What Are Medically Tailored Meals and How Do They Work?

A primary care doctor, registered dietitian (RD), nutritionist, or other professional provides medically tailored meal plans that address any special dietary requirements a person may have during early recovery after they discharge from the hospital.

Home-delivered meals and other nutritional services support recovery from specific health conditions, including heart disease, diabetes, lung disorders, and stroke. MTMs also help people of all ages recover more quickly from injuries, illnesses, and medical procedures.

LifeSpring Home Nutrition collaborates with clients or their care team to create individualized meal plans.

How Can Meal Preparation Services Help Seniors, Caregivers, and Family Members?

Meal preparation services simplify senior care by eliminating the need for grocery shopping and cooking while ensuring older adults have access to nourishing meals.

Seniors healing at home instead of in a hospice situation, care facility, or nursing home need nutritional support to avoid health complications after leaving the hospital. However, caregivers and family members may not know how to navigate meal preparation for seniors in recovery.

MTMs from LifeSpring Home Nutrition make meal preparation easy by providing pre-portioned, nutritionally balanced meals each week.

How Do Healthy Meals Support Mental Health and Energy During Recovery?

Healthy meals provide essential vitamins and minerals that regulate moods and support recovery.

Nutrients have a direct impact on gut health and cognitive function. MTMs ensure you consume enough essential nutrients to repair cells, strengthen your immune system, and improve neurotransmitter communication. You will feel more energized, focused, and positive during recovery if you eat a healthy diet.

How Do Medically Tailored Meals Support Medication Management?

Most people returning home from the hospital have medications to help them manage the healing process, and MTMs make it easier to adhere to medication guidelines.

MTMs introduce meal flexibility, promoting autonomy and reducing stress at home. Tailored meals support medication management by promoting overall health and wellness and creating a regular schedule.

Additionally, some prescription medications must be taken with food, and many people choose to take their medications at mealtime for convenience.

What Specialized Care Plans Include Nutrition for Post-Hospital Patients?

Medicare Advantage supplemental benefits, such as meal-delivery services, help people heal at home.

Multiple care plans, including Medicare Advantage, provide coverage for post-hospital nutritional support. You can check with your insurance provider today to determine if you qualify for home-delivered MTMs.

Specialized care plans give you the tools and resources to access essential recovery services while healing in the comfort of your own home.

Building a Personalized Nutrition and Care Plan

How Do Care Teams Assess Dietary and Health Needs After a Hospital Stay?

A care team uses assessments, screening tools, health history, and recovery goals to help people develop a dietary plan upon returning home.

Nutritional specialists, primary care doctors, and other professionals customize dietary guidelines based on the unique needs of each person. Meals tailored to recovery and individual health conditions often lead to better treatment outcomes and reduce the risk of rehospitalization.

How Does LifeSpring Coordinate With Family Members, Caregivers, and Social Workers?

LifeSpring Home Nutrition collaborates with families, caregivers, social workers, and medical professionals, connecting them with people who need additional support services or resources.

Post-hospital care often involves a variety of community-based services designed to ensure you have the resources to heal safely and comfortably.

LifeSpring Home Nutrition coordinates with families, caregivers, and community-based programs to ensure vulnerable populations have access to necessary care and nutritional support after returning home from the hospital.

How Does Nutritional Support Fit Into a Broader Post-Acute or In-Home Care Plan?

A post-acute or in-home care plan must address all a person’s daily needs, including nutritional support.

Proper nutrition reduces hospital readmissions and complications by ensuring the body gets the nutrients it needs to heal from the damage caused by illness, injury, or surgery. Meal planning and nutritional support become part of the daily routine that people must develop to regain their strength after a hospital stay.

How Does the Level of Care Affect Meal Planning After Hospital Discharge?

Short-term and post-acute care needs shape the form nutritional support takes during early recovery. For example, a person requiring a high level of care may need assistance with meal planning, preparing food, or eating.

The level of care you require after hospitalization will determine the types of foods and meal plan that best support your healing journey. For example, if you are recovering from a jaw surgery, you may need a specialized diet of pureed foods and shakes for several weeks or months.

LifeSpring Home Nutrition offers a wide range of food options, including specialized meals to suit any level of care.

How Is Nutrition Integrated Into Discharge Planning for Safe Recovery?

A medical professional provides comprehensive dietary guidelines designed to promote healing and reduce the risk of rehospitalization.

Before being discharged from the hospital, most people undergo a series of meetings with various members of the care team to ensure a safe recovery. Family members or caregivers may receive information and resources on how to provide the best dietary support to a loved one transitioning out of the hospital.

Meal planning provided before or during discharge ensures people in recovery know what foods to avoid and how to support their recovery.

Supporting Physical and Mental Health Through Meals

How Do Medically Tailored Meals Improve Physical Recovery and Energy Levels?

You will feel more energized and better able to overcome challenges in recovery if your body has the latest nutrients to stay healthy and revitalized.

The foods you eat play a role in hormone levels, immune system responses, gut health, and your general wellness. By eating healthy, balanced meals, you ensure your body has the nutrients it needs to recover and rebuild damaged tissue.

MTMs support your mental health, improve your mood, and enhance cognitive function.

How Does LifeSpring Help Seniors and Post-Hospital Patients Maintain Independence Through Easy Meal Access?

LifeSpring Home Nutrition helps seniors maintain a healthy diet and independence during the healing process by ensuring they have access to pre-packaged, easy-to-heat meals that meet their post-hospital nutritional needs

Seniors have a higher risk of rehospitalization during early recovery. Healthy eating impacts recovery from surgery, chronic illness, or behavioral concerns. Stress and meal planning often interact, making it difficult for seniors or caregivers to decide which foods provide the most nutrients.

We address special dietary needs with nutrition, and our meals are adapted to seniors. For example, older adults with dysphagia may follow strict pureed diets, and LifeSpring Home Nutrition offers multiple enriched pureed meal options.

How Does Nutrition Support Recovery for Patients With Diabetes, Renal Issues, or Heart Conditions?

People living with diabetes, renal issues, or heart conditions use diet to manage various aspects of their condition and limit the potential side effects or complications.

LifeSpring Home Nutrition offers meal plans for specific health conditions, including diabetes, renal issues, and cardiovascular disease. The meals reduce added sugars, sodium, and saturated fats to promote a healthier diet.

Many people find it easier to manage chronic medical conditions if they eat MTMs designed to address their specific needs.

Planning for a Safe Return Home

How Are Meals Delivered and Stored Safely for Recovery?

Home-delivered meals from LifeSpring Home Nutrition arrive once a week and are stored frozen.

The convenience of meal delivery services makes them ideal for people returning home after a short or long hospital stay. Frozen meals can be stored safely for months and reheated quickly for a nutritious and delicious meal.

How Do Daily Routines Integrate Healthy Eating With Physical Therapy, Occupational Therapy, or Skilled Nursing Care?

Daily routines keep people accountable for their activities and make it harder to forget important appointments and meals.

Care teams encourage daily routines and healthy eating habits to complement post-hospital recovery services, such as physical therapy.

Older adults may struggle to remember to eat or attend appointments. However, a regular schedule provides them with touchstones to stay on track and maintain a healthy diet and self-care.

How Can Family Caregivers Support Nutrition and Recovery at Home?

Family members often play a critical role in ensuring their loved one follows their discharge diet during post-hospital care by modeling healthy eating.

Families also support nutrition and recovery at home by doing the following:

  • Removing unhealthy foods or drinks from the home
  • Assisting with meal planning
  • Signing up for meal delivery services
  • Creating healthy meals for the whole family
  • Assisting with meal preparation and clean up

People recovering at home may feel tempted to return to old eating habits. However, avoiding processed foods, added sugars, high-sodium diets, and other unhealthy foods can speed healing.

Family members can help by removing tempting foods and replacing them with healthy alternatives that fit with the dietary guidelines provided by medical professionals.

How Do Follow-Up Appointments Ensure Proper Nutrition During Recovery?

Follow-up appointments allow medical professionals to assess progress and adjust dietary guidelines at each stage of healing.

Different levels of care may have specific dietary requirements. For example, as people heal and progress in their recovery, dietary requirements may become less strict or change to accommodate new symptoms and side effects.

Regular follow-up appointments ensure medical professionals can assess current health to determine if any dietary changes may be needed.

Choosing the Right Post-Hospital Nutrition Support

How Do Medically Tailored Meals Complement Skilled Nursing, Home Care, or Assisted Living Services?

Medically tailored meals provide essential nutrition, while skilled nursing and other assisted services provide the emotional and practical support needed to promote recovery.

Healing after hospitalization involves addressing medical, nutritional, and emotional needs. MTMs complement recovery care by supporting the body.

According to Healthcare, “MTMs are a cost-effective and common-sense solution” that “alleviate food and nutrition insecurity, assist in managing the leading causes of disease.” In addition, ”They also provide social benefit to both patients and caregivers as they seek to remain healthy.”

LifeSpring Home Nutrition makes it easy for families and caregivers to choose the right meals for supporting lasting recovery.

How Do Medicare, Medicaid, or Insurance Support Access to Post-Hospital Nutrition Services?

Most insurance plans cover post-hospital nutritional services, such as MTMs from LifeSpring Home Nutrition.

Check with your insurance provider to see if your plan covers nutritional support and delivery services. You can also call our office, and one of our dedicated support agents will help you determine your eligibility.
